Licensed Practical Nurse

📋 Description

• Making home visits to clients in designated territories.
• Administering medications, wound care, treatments, and procedures.
• Monitoring clients' conditions; report changes to Clinical Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary; case management and coordination.
• Document observations and interventions in EMR on a touch pad tablet.

🎯 Requirements

• Current NJ LPN license.
• Minimum 1 year LPN experience.
• Graduation from accredited nursing program (transcript or diploma).
• Home care experience preferred but not required.
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
