📋 Description

• Visit clients at home to aid recovery after injury or illness.
• Case manage and develop care plans; deliver skilled care (assessments, meds).
• Progress clients from discharge to meeting goals at home.
• Provide education to patients and families.

🎯 Requirements

• Active and current LPN license.
• One year of clinical experience preferred.
• Home health experience welcomed, but training provided.
• Ability to travel locally to cases as assigned.
