Licensed Practical Nurse - Home Health

📋 Description

• Making home visits to clients in designated territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor clients' conditions and report changes to Clinical Manager and Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Document observations, interventions, and evaluations in EMR on a state-of-the-art touch pad tablet.

🎯 Requirements

• Vermont LPN license.
• At least 1 year of recent LPN experience.
• Graduation from an accredited nursing program.
• Prior home care experience preferred.
• Ability to work independently and manage time; strong interpersonal skills.
• Solid computer skills; EMR experience preferred; ability to travel.
