📋 Description

• Making home visits to clients in designated territories.
• Administer medications, wound care, treatments.
• Monitor client conditions and report changes to Clinical Manager.
• Follow up on doctors' orders and document them.
• Perform client assessments and coordinate case management.
• Document observations and care in EMR on a touchpad tablet.

🎯 Requirements

• Current Licensed Practical Nurse license in Delaware (DE).
• Minimum 1 year of LPN experience.
• Graduation from an accredited nursing program.
• Prior home care experience preferred.
• Independent worker with strong time management; travel to cases.
• Strong interpersonal skills; proficient with EMR.
