📋 Description

• Making home visits to clients in designated territories.
• Administer medications, wound care, treatments and procedures.
• Monitor client conditions and report changes to supervisors.
• Document and follow doctors' orders accurately.
• Conduct client assessments and coordinate care.
• Use EMR tablets to document care.

🎯 Requirements

• Current LPN license in DE.
• Minimum 1 year LPN experience.
• Graduate of accredited nursing program.
• Prior home care experience preferred.
• Ability to travel to cases as assigned.
• Strong interpersonal skills; EMR experience.
