📋 Description

• Make home visits to clients in Lexington, VA.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to Clinical Manager.
• Follow doctors' orders and document care using EMR.
• Perform client assessments and coordinate case management.
• Utilize EMR on a tablet.

🎯 Requirements

• Home Health experience required.
• At least 1 year of LPN clinical experience.
• Active VA LPN license.
• Graduation from accredited nursing program.
• Prior home care experience preferred.
• Ability to travel to cases as assigned.
