Licensed Practical Nurse - Home Health (Full Time)

📋 Description

• Visit clients in their homes to aid recovery.
• Case manage and develop care plans for patients.
• Deliver skilled care: assessments, meds, wound care.
• Help clients progress toward home goals after discharge.
• Provide education to patients and families.

🎯 Requirements

• Active and current LPN license
• One year clinical experience preferred
• Prior home health exp welcome; we will train you
• Ability to travel locally to cases
