Licensed Practical Nurse (LPN)

📋 Description

• Ensure each individual receives quality, individualized health care management.
• Conduct routine site visits, check vitals, and perform assessments.
• Counsel on medication administration and document progress.
• Act as health/medical resource and train staff in medical arena.
• Conduct audits and reviews of medications.
• Ensure compliance with state requirements and company policies.

🎯 Requirements

• Current LPN license in State of Indiana required.
• 1-2 years experience with disabilities or long-term care preferred.
• Skilled, flexible, self-motivated, dependable, cooperative.
• Strong verbal/written communication skills.
• Ability to work independently and efficiently.
• Organizational and computer skills; acceptable background and driving record.
