Licensed Practical Nurse, LPN, Home Health Visits

📋 Description

• Make home visits to clients in Erie County on a per-visit basis.
• Provide skilled nursing and rehabilitative care as part of a multi-disciplinary team.
• Administer medications, wound care, treatments and procedures.
• Monitor client conditions and report changes to the Clinical Manager.
• Follow up on doctors' orders and document care accurately.
• Perform client assessments and participate in case management with EMR on a tablet.

🎯 Requirements

• Current PA LPN license.
• 1 year recent LPN experience.
• Graduate of accredited nursing program.
• Prior home care experience preferred.
• Ability to work independently and manage time.
• Strong interpersonal skills; EMR experience preferred.
