Making home visits to clients in designated territories.
Administer medications, wound care, and procedures.
Monitor client conditions; report changes to Clinical Manager and Client Services Manager.
Follow up with, execute, and document doctors' orders.
Perform client assessments and case management coordinating care.
Accurately document observations and interventions in EMR on a touch pad tablet.

🎯 Requirements

One year of clinical LPN experience.
Current LPN license in the state of OH.
Graduation from accredited nursing program.
Prior home health experience strongly preferred.
Ability to travel to cases as assigned.
Strong interpersonal and computer/EMR skills.
