Licensed Practical Nurse, LPN - Home Health Visits

📋 Description

• Making home visits to clients in designated territories.
• Administering meds, wound care, treatments, and procedures.
• Monitoring conditions; report changes to Clinical Manager.
• Following up and documenting doctors' orders.
• Case management and coordination.
• Documenting observations using EMR on a touch pad tablet.

🎯 Requirements

• A current Licensed Practical Nurse license in South Carolina.
• At least one year of recent LPN experience.
• Graduation from an accredited nursing program (transcript/diploma).
• Prior home care experience preferred.
• Ability to work independently and manage time effectively.
• Solid computer skills with EMR experience preferred.
