Licensed Practical Nurse, LPN - Home Health Visits

📋 Description

• Making home visits to clients in designated territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor clients' conditions; report changes to Clinical Manager.
• Follow up on doctors' orders and document them.
• Perform client assessments as necessary.
• Case management and coordination; document care in EMR on a touch pad tablet.

🎯 Requirements

• A current Licensed Practical Nurse license in North Carolina.
• Minimum of one year of recent, verifiable LPN experience.
• Home health visits experience required.
• Graduation from an accredited nursing program (transcript or diploma).
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
