Provide skilled nursing visits to clients in Hudson County, NJ.
Administer medications, wound care, treatments, and physician-ordered nursing interventions.
Monitor client conditions and report changes to the Clinical Manager.
Complete comprehensive nursing assessments as needed.
Document physician orders accurately and timely.
Coordinate care with physicians and the interdisciplinary team.

🎯 Requirements

Current New Jersey LPN license.
Minimum one year of recent, verifiable clinical LPN experience.
Graduate of an accredited nursing program.
Home health experience preferred but not required.
Ability to work independently and manage time effectively.
Strong communication, organizational, and interpersonal skills.
