Licensed Vocational Nurse (LVN)

📋 Description

• Provides direct nursing care and treatments within scope of practice and under appropriate clinical
• Conducts participant triage, nursing observations, and assigned assessments using established
• Implements individualized care plan interventions and supports chronic disease management, intake
• Coordinates participant care and transitions across the clinic, day center, home, external
• Partners with participants, caregivers, RN Case Managers, providers, therapists, social workers
• Provides participant and caregiver education on medications, nutrition, hygiene, chronic condition

🎯 Requirements

• Completion of an accredited vocational or practical nursing program, or equivalent nursing
• Active, unrestricted Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license, as
• Minimum of one (1) year of nursing experience, preferably supporting older adults, frail elderly
• Demonstrated ability to provide direct nursing care, recognize and communicate changes in
• Demonstrated ability to apply established clinical protocols while exercising sound judgment and
• Strong communication and collaboration skills, with the ability to effectively engage participants
