LPN - Transition Team

📋 Description

• Immediate LPN opportunities on BAYADA Transition Team for home care.
• Care for infants, children, and adults at home in Philadelphia.
• Support transitions from hospital/HICU to ongoing home care.
• Partner with CHOP to care for rare and complex diseases.
• Deliver quality care and provide training to the BAYADA care team.
• Overnight shifts (Mon-Sun) and weekend days available.

🎯 Requirements

• Have a current license as a Licensed Practical Nurse.
• NICU, PICU, ICU, or home care trach/vent experience.
• Pass high-tech Adult and Pediatric simulations.
• Hold current CPR certification.
• Ability to work in a teaching setting to educate the BAYADA care team.
• Availability to commit to an ongoing, consistent schedule.
