LPN - Transition Team

📋 Description

• Deliver quality home health care; support hospital-to-home transitions.
• 40 hours/week; day or overnight shifts.
• Care for infants, children and adults with rare/complex diseases.
• Travel to client homes in Chester, Delaware, and Montgomery counties.
• Train and educate the ongoing BAYADA care team as part of Transition Team.

🎯 Requirements

• NICU, PICU, ICU, or home care trach/vent experience required.
• Pass high-tech adult and pediatric simulations.
• Hold current CPR certification.
• Ability to teach and educate BAYADA care team.
• Availability to commit to a 40-hour weekly schedule.
• Flexibility to cover weekends - at least two weekend shifts per month.
