LPN - Transition Team

📋 Description

• Deliver high-quality home care during hospital-to-home transitions.
• Full-time, 40 hours per week on the Transition Team.
• Support transitions from hospital to ongoing BAYADA care.
• Learn to care for clients with rare and complex diseases.
• Provide training to the ongoing BAYADA care team.
• Day and overnight shifts available; choose schedule.
• Travel across Chester, Delaware, and Montgomery Counties.

🎯 Requirements

• NICU, PICU, ICU, or home care trach/vent experience required
• Pediatric trach/vent experience required
• Hold current CPR certification
• Availability to commit to 40 hours per week
• Weekend availability; at least two weekend shifts per month
• Ability to travel throughout Chester, Delaware, and Montgomery Counties
