LPN - Transitional Care Team

📋 Description

• Deliver quality home care to clients at home after hospital discharge.
• Coordinate transition from hospital/HICU to BAYADA home care.
• Partner with CHOP and care for rare, complex diseases.
• Train ongoing BAYADA care teams for continuity.
• Day and overnight shifts available, Mon-Sun.

🎯 Requirements

• Current LPN license (state-specific).
• NICU/PICU/ICU or home care pediatric trach/vent experience required.
• CPR certification current.
• Ability to travel throughout Philadelphia County.
• Ability to educate the ongoing BAYADA care team.
• Availability for an ongoing, consistent schedule.
