• Deliver coordinated, patient-centered virtual care management through telephone and messaging to
• Develop meaningful, individualized care plans in partnership with members and multidisciplinary
• Support members throughout fertility planning, prenatal care, delivery preparation, and postpartum
• Help members understand complex medical conditions, treatment pathways, healthcare benefits, and

🎯 Requirements

• Bachelor of Science in Nursing (BSN).
• Residence in a compact NLC state.
• Active Compact RN license in the state of residence, maintained in good standing with the
• Additional active nursing license in Illinois, California, Minnesota, or New York, a plus.
• Certified Case Manager (CCM) certification preferred.
• At least 5 years of nursing experience.
