Medical Social Worker

📋 Description

• Conduct in-home psychosocial assessments and clinical social work evaluations.
• Develop individualized intervention plans based on client and family needs.
• Assess social, emotional, financial, and environmental factors affecting health.
• Connect clients and families with community resources and supports.
• Advocate for clients and families navigating healthcare and community systems.
• Collaborate with nurses, therapists, physicians, and other team members to plan care.

🎯 Requirements

• Current New Jersey LMSW or LISW license.
• Master's degree in Social Work from CSWE-accredited program.
• Minimum of two years of community-based social work experience.
• Home health experience preferred, but not required.
• Strong communication, interpersonal, and organizational skills.
• Ability to work independently and effectively manage time.
