📋 Description

• Conduct in-home psychosocial assessments and social work evaluations.
• Develop individualized intervention plans for clients and families.
• Assess social, emotional, financial, and environmental factors affecting health outcomes.
• Connect clients with appropriate community resources and support services.
• Advocate for clients navigating healthcare and community systems.
• Collaborate with nurses, therapists, physicians, and other team members.

🎯 Requirements

• Active New Jersey LSW or LCSW license.
• MSW from CSWE-accredited program.
• Minimum of two years of community-based social work experience.
• EMR experience preferred.
• Strong communication, interpersonal, and organizational skills.
• Reliable transportation and ability to travel throughout Somerset and Union Counties.
