Medical Social Worker, Home Health

📋 Description

• Assist team in understanding social and emotional factors related to clients’ health
• Provide clinical social evaluations and plan interventions based on findings
• Identify community resources to address client and family needs
• Advocate for clients to navigate the community system
• Participate in the total plan of care with BAYADA clinicians
• Develop and maintain contacts with public/private agencies for patient resources

🎯 Requirements

• Master’s in social work with at least 2 years of community-based experience
• CSWE accreditation evidenced by transcript or degree
• Home health care experience a plus
• Solid computer skills; EMR experience preferred
• Ability to travel to cases as assigned
