Medical Social Worker, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Assist our team in understanding social and emotional factors related to clients’ health problems.
• Provide clinical social evaluations and plan interventions based on findings.
• Identify community resources to address client and family needs.
• Advocate for clients and families to navigate the community system.
• Participate in developing the total plan of care with BAYADA clinicians.

🎯 Requirements

• Licensed by the STATE Board of Social Work Examiners at LMSW or LISW level.
• MSW with at least 2 years of community-based experience.
• Accreditation by CSWE, demonstrated by transcript or degree.
• Home care experience a plus; strong English communication; ability to travel.
• Solid computer skills; EMR experience preferred.
• Must be able to work independently and manage time effectively.
