📋 Description

• Make home visits to clients in designated territories.
• Assess social and emotional factors affecting health.
• Provide social evaluations and plan interventions.
• Identify community resource referrals for patient and family needs.
• Advocate for patients to navigate community services.
• Collaborate in development of total care plans with BAYADA team.

🎯 Requirements

• Master of Social Work (MSW) and licensed LMSW/LISW in the state.
• 2+ years of community-based experience preferred.
• CSWE accreditation via transcript/degree.
• Home care experience a plus; strong English communication.
• Proficient in EMR; able to travel to cases as needed.
• Strong interpersonal and time-management skills.
