📋 Description

• Make home visits to clients in designated territories.
• Assess social and emotional factors related to health.
• Provide social evaluations and plan interventions.
• Identify community resources for client/family needs.
• Advocate for client and family through the community system.
• Document observations and interventions using EMR on a tablet.

🎯 Requirements

• Licensed by TN Board of Social Work Examiners at LMSW or LISW.
• MSW with at least 2 years of community-based experience.
• CSWE accreditation demonstrated by transcript or degree.
• Home care experience a plus.
• Strong English communication; read, write, and speak clearly.
• Ability to travel to cases as assigned.
