Medical Social Worker, Hospice

📋 Description

• Make home visits to clients in designated geographic territories in Salisbury, NC area.
• Assist the care team in understanding social and emotional factors related to client health
• Provide clinical social evaluations and plan interventions based on evaluation findings.
• Identify community resource referrals for client and family needs.
• Advocate for client and family to navigate the community system.
• Participate in the development of the total plan of care with BAYADA clinicians.

🎯 Requirements

• Masters in Social Work with at least 2 years of community-based experience.
• Accreditation by the Council on Social Work Education (CSWE).
• Home care experience a plus.
• Strong English communication; ability to travel to cases as assigned.
• Solid computer skills; EMR experience preferred.
