📋 Description

• Make home visits to clients in designated geographic territories.
• Assess social and emotional factors related to clients' health problems.
• Provide clinical social evaluations and plan interventions.
• Identify community resource referrals for client and family needs.
• Advocate for client and family through the community system.
• Participate in care plan development with the multidisciplinary BAYADA team.

🎯 Requirements

• Master's in Social Work with at least 2 years community-based experience.
• Accreditation by CSWE (Council on Social Work Education).
• Home care experience a plus.
• English: read, write, and communicate effectively.
• Travel to cases as assigned; work independently and manage time.
• Solid computer skills; EMR experience preferred.
