Medical Social Worker, Visits

📋 Description

• Assist team in social and emotional factors affecting clients’ health
• Provide clinical social evaluations and plan interventions
• Identify community resources for client and family needs
• Advocate for clients to navigate community resources
• Participate in total care plan development with BAYADA team
• Develop and maintain agency contacts for patient resources

🎯 Requirements

• Master’s degree in social work with CSWE accreditation
• At least 2 years of community-based social work experience
• Home Health Care experience a plus
• Solid computer skills; EMR experience preferred
• Able to travel to cases as assigned
