New Grad Physical Therapist, Per Diem Weekends, Home Health

📋 Description

• Make home visits to clients in designated territories.
• Perform mobility tests and measurements (ROM, balance, gait).
• Implement wound care and protective dressings/topical agents.
• Develop and adjust individualized care plans (manual therapy, gait, balance).
• Assess and revise PT care plan; work with nursing on care plan.
• Educate clients and families in rehabilitative care.
• Document observations and interventions on a tablet.

🎯 Requirements

• Active and current PT license in good standing.
• New grads welcome; strong interest encouraged.
• Willing to travel locally to cases.
• Home health experience a plus, not required; training provided.
