• Visit clients in their homes to support recovery.
• Perform mobility, strength, balance and safety assessments.
• Develop and adjust individualized PT care plans.
• Provide manual therapy and gait training as needed.
• Document care and progress on a tablet.
• Collaborate with nursing on multidisciplinary care plans.

• Active and current PT license.

• Ability to travel locally to cases as assigned.
