Nurse Practitioner (Remote, SC License Required)

📋 Description

• Cover our care needs for our Transitions of Care program
• Perform telephonic outreach and scheduled video/phone visits for patients post-hospital discharge
• Conduct medication reconciliation, symptom screening, and order needed follow-up labs, medications
• Build trust with patients, explain Galileo's care model, and connect them to services
• Coordinate care across settings including hospital and ED transitions, pharmacies, and community
• Collaborate with the nursing team

🎯 Requirements

• Certified Nurse Practitioner in Adult, Family, or Geriatrics
• Active state licensure in SC, with the ability to obtain additional state licensures
• Bonus if active licenses in TX, PA, IL, NY, MA, MN, LA, and/or NJ
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
• 2 of those years may be substituted by time spent working at a Skilled-Nursing facility or Long
• Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions
