Nurse Practitioner, Transitions of Care

📋 Description

• Cover our care needs for our Transitions of Care program.
• Day to day work includes telephonic outreach (“cold calls”) and scheduled video/phone visits for
• Visits focus on: Medication reconciliation, symptom screening, and ordering of any needed follow-up
• You will also navigate conversations with patients to build trust, explain Galileo's care model and
• Coordinate care across settings — hospital and ED transitions, pharmacies, and community providers
• Collaborate with our nursing team

🎯 Requirements

• Certified Nurse Practitioner in Adult, Family or Geriatrics
• Active state licensure in IL, with the ability to obtain additional state licensures
• Bonus if you have active licenses in TX, PA, NJ, NY, MA, MN, LA, and/or SC
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
• 2 of those years may be substituted by time spent working at a Skilled-Nursing facility or Long
• Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions
