📋 Description

• Develop, manage, and grow Wellness Coordination services to support strong clinical outcomes and
• Supervise, support, and manage nursing staff while promoting accountability, consistency, and
• Ensure the completion and/or review of Wellness Assessment, Risk Mitigation Tool, and Wellness Plan
• Ensure face-to-face visits are completed and documented in accordance with the DDRS Wellness

🎯 Requirements

• Active Indiana RN License: Must be a Registered Nurse with a current, unrestricted license in
• Strong Communication Skills: Ability to clearly document care, interpret and follow healthcare
• Supervisory Experience: At least 1 year of experience leading, supervising, coaching, or developing
• Valid Driver’s License: Must have a valid driver’s license and be comfortable traveling between
• Successful Background Clearance: Employment contingent upon successful completion of all required
• IDD/DD Nursing Experience: Prior nursing experience supporting individuals with intellectual

• Compensation: $71,335.76 Annually Fixed Rate plus $5,000 retention bonus
