• Program Development & Growth: Develop, manage, and grow Wellness Coordination services to
• Nursing Leadership: Effectively supervise, support, and manage nursing staff while promoting
• Wellness Assessments & Care Planning: Ensure the completion and/or review of the Wellness
• Required Visits & Documentation: Ensure face-to-face visits are completed and documented in
• Mission-Driven Leadership: Provide leadership and direction that supports the organization's
• Quality of Care: Promote consistency and quality in healthcare services provided to individuals

🎯 Requirements

• Active Indiana RN License
• Strong Communication Skills
• Supervisory Experience (at least 1 year)
• Valid Driver's License
• Successful Background Clearance

• Compensation: $71,335.76 Annually Fixed Rate plus $5,000 retention bonus
• Work Environment: Onsite in either Bloomington, Seymour or Evansville
