NY Licensed Bilingual Spanish/English Clinician - LMSW or LCSW - Behavioral Health

📋 Description

• Provides care to patients, based on needs of assigned Care Pod, customizing treatment plan to
• Supports and closely coordinates behavioral health care with the patient’s primary care provider
• Screens patients for common behavioral health and substance use disorders using PHQ9, GAD7
• Monitors patients for changes in clinical symptoms and treatment side effects/complications, with a
• Supports psychotropic medication management prescribed by PCPs, focusing on treatment adherence
• Tracks patient follow-up and clinical outcomes using a registry, records encounters in the registry

🎯 Requirements

• Master’s Degree in Social Work, Counseling, or related field from an accredited program
• Licensed to practice in the state of New York as a clinician (LMSW, LCSW or equivalent)
• Minimum 1-5 years experience as a licensed professional
• A secure, high-speed internet connection capable of supporting video conferencing and cloud-based
• Proficient in medical interpretation and documentation in English and Spanish that meet industry
