Occupational Therapist

📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status (muscle, endurance, vision, ADL/IADL).
• Assess cognitive, psychological and motor performance; create plans.
• Evaluate home environment for hazards to independence.
• Identify equipment needs and environmental adaptations.
• Revise care plan with nursing in multidisciplinary team.
• Educate patients, families in rehab and independent living.
• Document observations and evaluations using EMR tablet.

🎯 Requirements

• Current New Jersey Occupational Therapist license.
• Home care experience preferred, but not required.
• Minimum one year of OT experience.
• Graduation from an AMA and AOTA accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.
