Occupational Therapist

📋 Description

• Provide home visit occupational therapy services in assigned territories.
• Evaluate functional abilities including ADLs/IADLs.
• Develop individualized treatment plans to maximize independence.
• Assess home safety; suggest modifications or devices.
• Identify assistive devices and environmental adaptations.
• Collaborate with nursing/interdisciplinary team.
• Educate patients and families on therapy techniques for safety.
• Monitor progress and adjust plans as needed.
• Document evaluations and progress in EMR/tablet.

🎯 Requirements

• Current New Jersey OT license.
• Minimum 1 year OT experience.
• Home health experience preferred but not required.
• Graduate of accredited OT program.
• Strong assessment, communication, organizational, problem-solving.
• Ability to work independently and manage time.
• EMR experience preferred.
