Occupational Therapist

📋 Description

• Provide skilled home visits in designated territories.
• Evaluate functional abilities: ADLs, IADLs, mobility, cognition, communication, home safety.
• Develop individualized treatment plans to improve independence.
• Assess home environments; recommend adaptive equipment and modifications.
• Collaborate with nurses, therapists, physicians, and other professionals.
• Educate patients and families on interventions and safety; document in EMR.

🎯 Requirements

• Current New Jersey OT license.
• Graduate of an accredited Occupational Therapy program.
• Minimum of one year of recent OT experience.
• Strong clinical, organizational, communication, and problem-solving skills.
• Ability to work independently and effectively manage time.
• EMR documentation experience preferred.
