Occupational Therapist

📋 Description

• Provide one-on-one OT services to adults in patients' homes.
• Conduct comprehensive functional assessments (ADLs, IADLs, mobility).
• Develop and implement individualized treatment plans to maximize independence and safety.
• Assess home environments; recommend adaptive equipment and modifications.
• Collaborate with nursing and interdisciplinary team to coordinate care.
• Educate patients and families on therapy techniques and home safety; document progress in EMR.

🎯 Requirements

• Active New Jersey OT license.
• Minimum 1 year OT experience.
• Home health experience preferred.
• Graduate of an accredited OT program.
• Strong clinical assessment and critical thinking.
• Willingness to travel throughout Monmouth, Middlesex, Somerset, and Union Counties.
