Occupational Therapist

📋 Description

• Perform comprehensive OT evaluations and develop individualized treatment plans.
• Provide outpatient therapy within senior living communities and group homes.
• Deliver evidence-based interventions to promote independence and safety.
• Collaborate with home health clinicians for smooth transitions of care.
• Educate clients, families, and caregivers on plans, home programs, and safety.
• Complete timely documentation and discharge planning using BAYADA EMR.

🎯 Requirements

• Current New Jersey OT license.
• Minimum 1 year of OT experience.
• Home health experience preferred, not required.
• Graduate from an accredited OT program (AOTA/AMA).
• Strong communication, organization, and time management.
• Ability to work independently with an interdisciplinary team.
