Occupational Therapist

📋 Description

• Provide skilled occupational therapy services through home visits within designated territories.
• Perform comprehensive evaluations of patients' functional abilities, including ADLs and IADLs.
• Develop individualized treatment plans to maximize independence, safety, and outcomes.
• Assess home environments for safety and recommend modifications or devices.
• Identify assistive devices and environmental adaptations to improve function.
• Collaborate with nursing and interdisciplinary team to coordinate care.

🎯 Requirements

• Current New Jersey OT license.
• Minimum of one year of OT experience.
• Home health experience preferred but not required.
• Graduate of an accredited OT program.
• Strong assessment, communication, and problem-solving skills.
• Ability to work independently and manage time.
