Occupational Therapist

📋 Description

• Make home visits to clients in designated territories.
• Conduct evaluations of functional status including ADL/IADL.
• Assess cognitive, psychological, and motor performance; plan interventions.
• Evaluate home environments for hazards and barriers.
• Identify equipment needs and adaptations for independence.
• Update care plans; collaborate with nursing in a multidisciplinary team.
• Educate patients and families in rehab activities for safety and independence.
• Document observations and interventions using a touchpad EMR system.

🎯 Requirements

• A current New Jersey Occupational Therapist license.
• Prior home care experience preferred, but not required
• A minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program (AMA/AOTA).
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills; problem solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
