Make home visits to clients in designated territories.
Evaluate functional status: muscle function, endurance, coordination, communication, ADL/IADL.
Assess cognitive, psychological, motor aspects; develop intervention plans.
Evaluate home environment for hazards/barriers.
Identify equipment needs and environmental adaptations.
Reassess and revise OT care plan; collaborate with nursing.

🎯 Requirements

VA OT license
One year OT experience; home care preferred
Graduate from an accredited OT program
Ability to work independently and manage time
Strong interpersonal, organizational, problem-solving skills
Solid computer skills; EMR experience preferred
