Occupational Therapist- Home Health

📋 Description

• Evaluate patients’ functional status, ADL/IADL, and endurance.
• Assess cognitive, psychological, and motor performance; plan rehab.
• Evaluate home environment for hazards to independence.
• Identify equipment needs and environmental adaptations.
• Revise OT care plans; collaborate with nursing in a multidisciplinary team.
• Educate patients and families in rehab activities to promote independence.

🎯 Requirements

• Be licensed as an Occupational Therapist in Hawaii.
• Must be willing to travel in assigned territory.
• Graduate from OT program accredited by AMA and AOTA.
• 1 year of OT experience preferred.
