Occupational Therapist- Home Health

📋 Description

• Make home visits to clients in designated territories.
• Evaluate functional status: muscle function, endurance, ADL/IADL, and communication.
• Assess cognitive, psychological, and motor aspects; develop intervention plans.
• Assess home hazards and environmental barriers to independence.
• Update care plan with the multidisciplinary team and document in EMR.
• Educate patients and families on rehab to promote health and independence.

🎯 Requirements

• Current Vermont Occupational Therapist license.
• Minimum 1 year recent OT experience.
• Graduation from an accredited OT program (AMA/AOTA).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
