Occupational Therapist- Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate functional status and ADL/IADL; assess endurance and coordination.
• Assess cognitive, psychological, and motor aspects; develop intervention plans.
• Evaluate home environment for hazards and independence barriers.
• Identify equipment needs and environmental adaptations to improve outcomes.
• Update care plan; document care and educate patients and families.

🎯 Requirements

• Vermont OT license.
• Prior home care experience preferred, not required.
• Minimum one year OT experience.
• Graduation from accredited OT program (AMA/AOTA).
• Ability to work independently; manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills; EMR experience.
