📋 Description

• Execute care plans from hospital discharge to safe, independent home life.
• Make home visits to clients in designated geographic territories.
• Evaluate functional status: muscle function, endurance, coordination, ADL/IADL.
• Assess cognitive, psychological and motor aspects; develop individualized plans.
• Evaluate home environment for hazards to promote safety.
• Identify equipment needs and environmental adaptations.
• Continually revise OT care plan; coordinate with nursing in multidisciplinary plan.

🎯 Requirements

• Active and current OT license in NJ.
• Minimum of one year of recent OT experience.
• Home healthcare experience welcomed; training provided.
• Ability to travel to cases as assigned.
• Calm, attentive with strong assessment and written/verbal communication.
• Compassionate, reliable patient care in home settings.
