Occupational Therapist - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate patients' functional status, including strength, endurance, vision, and ADL/IADL.
• Assess cognitive, psychological, and motor function; develop intervention plans.
• Evaluate home environments for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Educate patients and families; update care plan and coordinate with nursing.

🎯 Requirements

• A current VA Occupational Therapist license.
• A minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited jointly by the AMA and the AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior experience with EMR preferred.
