Make home visits to clients in designated territories.
Evaluate patients' functional status (muscle function, endurance, coordination, ADL/IADL).
Assess cognitive, psychological, and motor aspects; develop care plans.
Evaluate home environment for hazards and barriers to independence.
Identify equipment needs and environmental adaptations.
Document observations and revise the OT care plan with the care team.

🎯 Requirements

Current Pennsylvania Occupational Therapist license.
Home care experience preferred, but not required.
A minimum of one year of recent OT work experience.
Graduate of AMA/AOTA-accredited OT program.
Ability to work independently and manage time effectively.
Solid computer skills; EMR experience preferred.
