Occupational Therapist (OT) - Home Health

📋 Description

• Make home visits to clients in designated territories.
• Evaluate patients' functional status and ADL/IADL performance.
• Assess cognitive, psychological, and motor aspects of performance.
• Evaluate home environments for hazards or barriers.
• Identify equipment needs and environmental adaptations.
• Update and revise the care plan with the multidisciplinary team.

🎯 Requirements

• A current North Carolina Occupational Therapist license.
• Prior home care experience preferred, but not required.
• A minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program (AMA/AOTA).
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience.
